Rehabilitation enables smooth functioning of your everyday life at home. You can get home rehabilitation, for example, when you are discharged from a hospital. On this page we will tell you about home rehabilitation.
In rehabilitation, you will practice daily routines with a professional.
You will practice, for example,
- getting out of bed
- walking in the stairs
- visiting a shop.
You can get access to home rehabilitation via a health and social services professional. The professional will direct you to home rehabilitation. First, we will assess your situation at home. Then we will together make a plan for rehabilitation.
If you need home rehabilitation, please contact your healthcare unit or healthcare professional to obtain referral. Persons over the age of 65 who do not have another healthcare contact are asked to contact the Client Assistance for Older People
