Are you concerned about your own well-being? Are you worried about someone else? Then you can make a notification of concern. A notification of concern can help you or someone else to get help in a difficult situation
What is a notification of concern?
In the notification of concern, you can tell a social worker about your concern. The professionals will then contact the person you are worried about. Social workers offer help to the person you are worried about.
You can make a notification of concern about yourself or someone else. You do not need to ask permission to make the notification.
Make a notification of only one person at a time. If you want to make a notification of many people, make a separate notification of each person.

Once the notification of concern has arrived, social workers will look at the situation and provide help. Social workers are bound to secrecy. It means they are not allowed to tell outsiders about the matter. So, you will not be informed of the situation if you make a notification of concern about another person.
If you do not have permission to make a notification:
- Fill out the electronic form Notification of the Need for Social Services (you will transfer to another service)(siirryt toiseen palveluun)
- You can also open the form Notification of the Need for Social Services (in Finnish)(siirryt toiseen palveluun)
- Print out the form.
- Read the instructions and fill in the form.
- Send the form by mail to the address:

If you have permission to make a notification:
- Select either an electronic or printable version of the form:

- Print out the form if you chose the printable form.
- Read the instructions and fill in the form.
- Send the form by mail to the following address:
