Long-term diseases are diseases that are monitored and need to be treated for a long time. A disease is a long-term disease if it is treated for longer than six months. On this page we will tell you about the monitoring and treatment of long-term diseases.
Long-term diseases are monitored in periodic controls. A periodic control is a check where a professional examines the situation of your disease. With the help of control, we learn about your health and how the treatment of the disease works. We can change your medication, for example, if we find in the control that something is not working. It will help to keep the disease under control.
Long-term diseases can be monitored, for example, with
- laboratory tests
- surveys and self-assessments
- imaging examinations
How are diseases monitored?
A periodic control is done remotely via the HyVä digi chat if your disease does not require regular monitoring at the health centre. If you cannot use electronic services, the control can be made by telephone.
Some diseases are monitored on site at a health centre. Book an appointment for control at the health centre if your disease requires it. You will be invited to the control by HUS if the control needs to be done in specialised medical care.
This page provides information on how different diseases are monitored. However, a professional can make you a personal treatment plan. In that case you will be treated according to the plan. Always follow the treatment instructions you have received in your treatment plan. Make sure your disease is monitored according to the instructions.
You need to make sure yourself that your disease is monitored according to the instructions. Book an appointment for examinations according to the instructions. You will find instructions for treatment of various diseases later on this page. If you do not have a treatment plan, follow the instructions on this page.
Make sure you have a referral to the laboratory if laboratory tests are part of monitoring your disease. You can check the information via the HyVä digi chat or by calling the health centre. Have the laboratory tests if you have a referral to them.
Fill in the necessary questionnaires if your disease is monitored by tests. Write down the test results for yourself. You can also save the test results for yourself as a PDF file or take a picture of the results.

Monitoring asthma depends on whether your disease is balanced.

If you got 20 points or more in the Asthma Control Test
Your asthma is well balanced if you got more than 20 points in the Asthma Control Test. In that case you will have the periodic control every two years.

- your other diseases do not need to be monitored at the health centre.
If you cannot use electronic services, we can do the control by telephone.

If you got less than 20 points the Asthma Control Test
Your asthma is unbalanced if you got less than 20 points in the Asthma Control Test. In that case, you will have the periodic control every year. We will do the control on site at the health centre.

- Measure your peak expiratory flow (PEF) on seven days before the control.
- Take the reliever medication before you blow.
- Write down the results of the blows.

An early stage of diabetes or prediabetes means that your blood sugar is elevated.

Monitoring chronic obstructive lung disease depends on whether your disease is balanced.

You need a blood pressure monitor to measure your blood pressure.

Measure your blood pressure for four consecutive days. Measure your blood pressure both in the morning and evening:
- Measure blood pressure at 6.00–9.00 in the morning.
- Take your medicine only after measuring blood pressure.
- Measure your blood pressure at 18–21 in the evening.
- Make your measurements at about the same time each day.
Before you measure your blood pressure:
- Go to a peaceful, quiet, and warm place.
- Always measure your blood pressure in the same place.
- Always measure your blood pressure of the same arm.
- Half an hour before blood you measure your blood pressure:
- Do not eat.
- Do not smoke.
- Do not drink coffee or tea.
- Do not drink other stimulating drinks, like cola drinks.
- Do not drink alcohol.
- Do not exercise or exert yourself physically.
- Do not take your medication.
- Relax and keep still ten minutes before measuring your blood pressure.
Measure your blood pressure:
- Put a blood pressure monitor strap, or cuff, on your upper arm.
